Community Health Hubs

ONGOING PROJECT

Community Health Hubs (CHHs) are physical sites in the community that provide a broad range of health & wellness resources & services that respond to community-voiced needs.

We collaborate with community-based organizations & other local institutions to bring sought-out resources that are otherwise inaccessible by the community. CHHs activate community spaces by not only bringing together consistent health & wellness resources but also by incorporating a celebration of local culture through art, music, & play.

Offerings change weekly and can include complementalternative medicine services (e.g. yoga, massage, dance, art therapists, musicians, acupuncture, etc) delivered by practitioners of color, peer-to-peer government-benefit enrollment services, exercise classes, distribution of fresh grocery bags & meals, & healthy-eating demonstrations led by nutritionists of color.

CHHs are responsive to the needs & culture of the community—this is made possible through meaningful partnerships with community-led spaces by taking direction from community champions & investing in culturally specific programming that promotes community building.
